Apologies if this one is in the wrong place ..... I don't know enough to know where it should best go !!!

I want a simple multi-option questionnaire on my site that customers can complete in order to detail what they are looking for from me and which will, on completion, provide a total "estimated" price.

I figure I would need to put a price on every option in each question so that it can calculate total price.

Do I need a script for this or is it a database driven thing ...... and can anybody point me at software/scripts that will enable me to build this ?

Any help appreciated.
